School Ministries Alabama is honored to make Release Time Bible Education available for families with students attending this school. Our program provides an opportunity for students to step out during the school day to explore the message of the gospel, deepen their understanding of biblical truth, and discover hope that endures well beyond the classroom. Guided by a commitment to service, community, and respect for each family’s needs, this program is designed to be both accessible and enriching—empowering students with valuable life lessons in a supportive environment. I, the undersigned Parent/Guardian, give permission for my child ("student", named below) who attends Vincent Middle High School ("VMHS"), to participate in an Extended Learning Opportunity (“ELO”) Bible education class sponsored by School Ministries Alabama (“School Ministries”) during the 2026-2027 school year.

I understand that classes will be held 8am-9am every Tuesday morning in accordance with the Shelby County Schools 2026-2027 Academic Calendar, and will meet at First Baptist Church Vincent ("FBC Vincent"), 15 Waite Street, Vincent AL 35178. I understand that School Ministries will provide transportation for my student from VMHS to FBC Vincent for Bible class, and from FBC Vincent back to VMHS after Bible class.

I understand there may be occasions when photos or video may be taken during this ELO Bible class; I consent for School Ministries and its agents to use and publish, without charge, photos, videos, print material, or other media that may include images and words of or about my child when associated with School Ministries or the VMHS program.

I understand and agree that: (1) This class is designed to NOT conflict with required core classes, but that my student is still responsible for VMHS assignments that may be missed during ELO Bible classes. (2) This ELO Bible class will be placed in my student's school class schedule. This means that (a) I may withdraw my permission for my VMHS student to attend all future ELO Bible classes with at least 48-hours written notice to VMHS and School Ministries, and (b) if no such prior notice has been given to withdraw my student from all future ELO Bible classes, and I wish my student to avoid attending a scheduled class, I must notify VMHS that my student will be arriving to school later in the morning the day of Bible class. (3) School Ministries may withdraw my student from the class, with at least 48-hours written notice to me, my student's parent/guardian, and VMHS, if circumstances require (e.g., unresolved behavioral misconduct of and by my student).

I understand that any supporting educational accommodations or interventions provided to my student by VMHS or Shelby County School District are not available or otherwise provided to or for my student during their attendance at this specific ELO class, including students required to be supported by a school nurse or paraeducator during the VMHS school day. Food allergies do not require a school nurse to accompany your child, and the information you provide below is requested so that SMA can be aware and prepared if necessary.
