I, the undersigned Parent/Guardian, give permission for my child ("student", named below) who attends Elvin Hill Elementary School (EHES) to participate in an Extended Learning Opportunity (“ELO”) Bible education sponsored by School Ministries Alabama (“School Ministries”), with classes held during the 2026-2027 every second Thursday, starting September 10th, 2026, from 1:45pm-2:45pm at Columbiana Recreation Center ("Rec Center"), 194 Washington Street, Columbiana, Alabama, 35051. I understand that my student will walk from EHES to the Rec Center, but that I will be responsible for picking up my student at the Rec Center at 2:45pm after class. Students will NOT be returned to EHES after class, and School Ministries will NOT provide transportation to any student’s home or any other location after class.
Classes will meet every second Thursday according to the following schedule:
- Fall 2026 – 09/10, 09/24, 10/08, 10/22, 11/05, 11/19, 12/03, 12/17
- Spring 2027 (Tentative) - 01/14, 01/28, 02/11, 02/25, 03/11, 04/08, 04/22, 05/06, 05/20
I understand there may be occasions when photos or video may be taken during this ELO Bible class; I consent for School Ministries and its agents to use and publish, without charge, photos, videos, print material, or other media that may include images and words of or about my child when associated with School Ministries or the EHES program.
I understand and agree that:

This class is designed to NOT conflict with required core classes, but that my student is still responsible for EHES assignments that may be missed during ELO Bible classes.

This ELO Bible class will be placed in my student’s school class schedule. This means:

I may withdraw my permission for my EHES student to attend all future ELO Bible classes with at least 48-hours written notice to EHES and School Ministries, and
-
if no such prior notice has been given to withdraw my student from all future ELO Bible classes, and I wish my student to avoid attending a scheduled class, I must sign my child out of school at EHES before 1:15pm on the day of the scheduled class.

School Ministries may withdraw my student from the class, with at least 48-hours written notice to me, my student’s parent/guardian, and EHES, if circumstances require (e.g., unresolved behavioral misconduct of and by my student).
I understand that any supporting educational accommodations or interventions provided to my student by EHES or Shelby County School District are not available or otherwise provided to or for my student during their attendance at this specific ELO class, including students required to be supported by a school nurse or paraeducator during the EHES school day. Food allergies do not require a school nurse to accompany your child; however, that information is requested below so we are aware and prepared if necessary.
