I, the undersigned Parent/Guardian, give permission for my student enrolled at Sardis Middle School (SMS) to attend Extended Learning Opportunity ("ELO") Bible education sponsored by School Ministries Alabama ("School Ministries"), with classes held every Thursday, starting September 19th, 2024, from 12:35 to 1:45 pm at Sardis Baptist Church, 1501 Church Street, Boaz, Alabama, 35956. I understand that School Ministries will provide transportation from SMS to Sardis Baptist Church and then back to SMS after class.
Classes will meet every Thursday according to the following schedule:
Fall 2024 - 9/19, 9/26, 10/3, 10/10, 10/17, 10/24, 10/31, 11/7, 11/14, 11/21, 12/5, 12/12, 12/19
Spring 2025 - 1/9, 1/16, 1/23, 1/30, 2/6, 2/13, 2/20, 2/27, 3/6, 3/13, 3/20, 4/3, 4/10, 4/17, 4/24, 5/1, 5/8, 5/15

I understand there may be occasions when photos or video may be taken during this ELO Bible class; I consent for School Ministries and its agents to use and publish, without charge, photos, videos, print material, or other media that may include images and words of or about my child.

I understand and agree that:
1. This class is not designed to conflict with required core classes, and my student is responsible for school class assignments that may be missed during ELO Bible classes.

2. This ELO Bible class will be placed in my child's school class schedule. This means:
I may withdraw my permission for my SMS student to attend future ELO Bible classes with at least 48-hour written notice to School Ministries and SMS.

3. School Ministries may withdraw my student from the class, with at least 48-hour written notice to the student's parent/guardian and SMS, if circumstances (e.g., unresolved behavioral issues) require.
I understand that any supporting educational accommodations or interventions provided to my student by Sardis Middle School or the Etowah County School System are not available or otherwise provided to my student during their attendance at this specific ELO class, including students required to be supported by a school nurse or paraeducator during the school day. Food allergies do not require a School nurse to accompany your child; however, that information is requested below so we are aware and prepared if necessary.

Agreement Section
To participate in an extended learning opportunity (ELO), all parties must agree to the
following:

ALL PARTIES
1. Agree that the primary purpose of an ELO is educational and understand that the student
will use the experience to satisfy the criteria outlined in the Alabama graduation
requirements.
2. Understand that the experience and duties defined will be planned and managed by
the student.
3. Understand that complaints should be addressed to and resolved by a
designated school official and supervisor.
4. Agree that transportation to and from the ELO is the responsibility of the student.
5. Be informed of any termination, resignation or change in experience. In some
instances, these actions must be approved by the school official.
STUDENT
1. Upholds the policies, rules, and regulations of the school and business or
organization.
2. Ensures that his or her actions, attitudes, and appearance reflect positively on the
school, business, or organization.
3. Provides advance notice of any absence to the supervisor and school advisor.
4. Is responsible for personal conduct demonstrated at school and during the ELO
experience.
5. Leads the work that is outlined in the ELO agreement.
6. Documents progress toward and meets the criteria set forth in the rubric used to
evaluate progress.
PARENT/CAREGIVER
1. Encourages the student to carry out duties and responsibilities in a timely and
effective manner.
2. Ensures that the student experience is safe and healthy.
3. Regularly counsels the student regarding progress and ensures that identified
learning targets have been met.
4. Arranges transportation for the student to and from the ELO organization.
5. Holds the school and ELO organization harmless for risks associated with
transportation and indirectly monitored activities.
SCHOOL OFFICIAL
1. Monitors student progress at regular, agreed-upon intervals throughout the ELO.
2. Enforces policies, rules, and regulations.
3. Determines the student’s final grade based on assessments by the student, ELO
organization, and school supervisor’s criteria (see sample rubric).
4. Awards student credit for ELO experience.
